Fourth Dose of the COVID-19 Vaccine

A Fourth Dose of the COVID-19 Vaccine will start.

≪Who is eligible?≫

1	People ages 60 years and older who have completed three doses of the COVID-19 vaccination and at least five months have passed, and who wish to be vaccinated.
2	People ages between 18 and 59 years who have completed three doses of the COVID-19 vaccination and at least five months have passed, and who have certain pre-existing conditions and wish to be vaccinated. ※Please contact the call center for the issuance of the vaccination voucher.

The voucher will be sent out every week or every other week after the above schedule.

※It may take longer to receive a voucher depending on the mailing status.

When you receive your vaccination voucher, please make an appointment.  One dose of the vaccination will be administered, and it is free of charge.  
Please keep your vaccination voucher in a safe place until you receive your vaccination.

Please fill out the form and bring it to the vaccination site on the day of vaccination without detaching the left side.
The right side is the document certifies that you are vaccinated. Please keep it in a safe place after your vaccination has been completed.

   From June 17th to July 23rd
   Fridays and Saturdays  10am~4pm
   You can make an appointment in person at the desk.
   ※No interpreter available
   ※Please bring your Preliminary examination slip and vaccination voucher.

Notes

• You cannot get other vaccinations for two weeks before and after your vaccination for the COVID-19.
• There is no interpreter at a group vaccination site.  Please come with someone who speaks Japanese.  One interpreter and one caregiver are allowed to accompany you.
